Happy Teddy Bear appeared in the prequel game, Lobotomy Corporation, as a HE Abnormality, with a similar appearance, maintaining its large damaged teddy bear look. It would kill employees by strangulation/neck-breaking through a hug if they worked with it twice in a row.

Happy Teddy Bear's stage is composed of a large room in ruins, with missing pieces of the walls that show buildings in the background. The floor is made of big jigsaw puzzle pieces with blocks, with a large torn stuffed animal on the background. The walls are gray, with patterns of animals in red, and white colors.
